Abstract

Epigenetics has emerged as an important field in modern medicine, providing new insights into the regulation of gene expression and the development, progression, diagnosis, and treatment of human diseases. Unlike genetic mutations, epigenetic modifications can alter gene activity without changing the underlying DNA sequence. This review discusses recent advances in medical epigenetics, with particular emphasis on three major regulatory mechanisms: DNA methylation, histone modifications, and non-coding RNAs. These mechanisms interact with genetic, environmental, and cellular factors to regulate gene expression and may contribute to disease development across multiple biological systems. The review highlights the influence of environmental exposures, nutrition, lifestyle, stress, and early-life experiences on epigenetic regulation and their potential long-term effects on health. The role of epigenetic alterations in systemic disorders, cancer, cardiovascular and respiratory diseases, neurological conditions, immune-related disorders, and other multisystem diseases is also examined. Furthermore, advances in epigenetic biomarkers and bioinformatics, including machine-learning approaches, are discussed for their potential applications in disease diagnosis, prognosis, and precision medicine. Particular attention is given to pharmacological and therapeutic strategies targeting epigenetic mechanisms, including DNA methylation inhibitors and histone-modifying agents. Although challenges remain regarding treatment specificity, safety, toxicity, and the complex interaction between genetic and environmental factors, continuing advances in epigenomic technologies and personalized medicine are expanding the clinical potential of medical epigenetics. Overall, epigenetic research represents a promising approach for developing individualized strategies for disease prediction, prevention, diagnosis, and treatment.
